La Princesa had an ambitious mandate: participate in the European Health Data Space by making its clinical imaging datasets available to researchers and institutions across Europe. The challenge was doing it without compromising patient privacy or moving sensitive data outside the hospital's own walls.

TietAI built the entire infrastructure on-premise, inside the hospital network. Hydra Platform sits across two hospital servers, ingesting radiology studies directly from the PACS system, applying clinical-grade anonymisation, and transforming the data into the OMOP common data model. A live EHDS metadata catalog then makes those datasets discoverable across the European Health Data Space, without any patient data ever leaving La Princesa.

All AI inference runs on local models. There are no external cloud dependencies, no third-party AI providers, and no compliance exceptions to negotiate. The hospital's data sovereignty is absolute.

At Vall d'Hebron, one of Spain's largest university hospitals, physicians were spending a significant part of their working day on documentation. Filling SOAP notes, assigning ICD-10 codes, validating prescriptions. All of it after the consultation had already ended. And with patients arriving speaking Catalan and Spanish, no off-the-shelf transcription tool came close to fitting the real clinical environment.

TietAI is piloting a six-agent pipeline that activates with a single gesture at the start of a consultation. It transcribes in real time across languages, pulls relevant context from the patient's FHIR history, validates diagnoses and prescriptions against the national medicines database, and inserts a completed, structured clinical note directly into the hospital's SAP system, all before the doctor has reached for their keyboard.

Patient data is anonymised at the point of entry and never stored on TietAI infrastructure. The pilot runs in an isolated, ENS Alta certified tenant, meeting Spain's highest public sector security standard from day one.

Docline runs one of Spain's most widely used telemedicine platforms, connecting patients and clinicians across primary care and specialist services. At that volume, the burden of clinical documentation adds up fast. Doctors were finishing consultations and then spending valuable minutes writing up notes: unstructured, hard to integrate, and disconnected from the downstream systems that depend on them.

TietAI Scribe is now embedded directly in Docline's consultation workflow. As each video call ends, a structured clinical note is already written, validated, and ready to integrate with Docline's platform and connected health systems. Clinicians stay focused on the patient.
